State Medicaid managed care contracts were reviewed for language on social determinants of health (SDoH) activities across 10 contractual domains. The authors then developed case studies of MCO opportunities and challenges in 5 states (California, Colorado, Georgia, Illinois, and Pennsylvania) based on interviews with officials and Medicaid managed health plans across community health centers, Primary Care Associations, and Community-Based Organizations.
